    Alliance for Nonprofit Resources, (ANR) is currently seeking Community Health Workers (CHW) to join our growing team! The open positions are for full-time and part-time. This person will be responsible for providing ongoing assessment, case management, care coordination, and monitoring of the Community To Home (C2H) participants. This person will also be responsible for outreach within their regions to promote the C2H program and stay connected with resources to best serve C2H participants.  

    The Northwest Central Region covers the following counties and the Community Health Worker(s) for this region should reside within the region or a county adjacent:  McKean, Potter, Elk, Cameron, Jefferson, Armstrong, Indiana, Cambria, and Clearfield.

    • Daylight, Monday-Friday

    • Full-time employees must work 40 hours/week.  Part-time employees must work 20 hours/week

    • This position is grant funded (Community to Home Grant) for 3 years with the possibility of extension for up to 2 additional years

    • This position will be fully remote.  Great for work/life balance!

    • Travel required within the regions above.  Travel mileage reimbursement is available at the state set rate

    • Hourly rate is $21.00/hr.

    Responsibilities:

    • Conduct comprehensive assessments of consumers to determine needs.

    • Develop a comprehensive service plan for persons determined to be in need of service.

    • Ensure continuity of the services for the consumer and coordinate the services received.

    • Normal office setting with frequent computer work approximately 25% of the time.

    • Travel to community sites, agencies and consumer homes with varying degrees of temperature, humidity and noise approximately 75% of the time in a variety of terrains and weather conditions.

    • Other duties as assigned to fulfill the mission of the agency.

    Qualifications:

    • Must be over the age of 18

    • Must have an associates degree and 2 years of relevant experience.  If no degree, must have 5 years of relevant experience

    • At least 3 years’ experience working in a community setting.

    • Must have a CCHW certification or be able to obtain the certification within one year of employment.

    • Knowledge of resources and demonstrates the ability to access resources in Pennsylvania

    • Maintain strict confidentiality

    • Valid driver’s license, proof of auto insurance.

    • Act 33/34 and FBI clearances will be required.  EOE/ADA

    Position Summary:

    Come join our team and our growing Certified Investigation Department in Allegheny County!  Alliance for Nonprofit Resources (ANR) is currently seeking a professional and self-motivated individual looking to make a difference.  Certified Investigators are responsible for completing community-based investigations for allegations of harm or abuse to individuals with intellectual or developmental disabilities.

    Must be currently or willing to become a Certified Investigator through the PA Office of Developmental Programs.  ANR will pay for trainings.  Must be able to work independently, think critically, be highly organized, willing to travel regionally, and operate within mandated timeframes.

    Essential Functions of Job:

    • Maintain current Certified Investigation status with Office of Developmental Programs (ODP).

    • Maintain knowledge of the HCSIS, MyODP, EIM and E-System systems.

    • Available and able to respond and engage investigation process within 24 hours.

    • Understanding and implementing the Certified Investigative process independently.

    • Serve as the lead Certified Investigator for assigned area but will provide back up support to other areas as needed.

    • Ability to identify witnesses and conduct thorough and objective witness interviews.

    • Ability to compile all relevant evidence into a comprehensive Certified Investigation Report.

    • Ensure all forms of documentation are gathered to a complete evidence file.

    • Maintain the highest level of ethical behavior and professionalism while conducting certified investigations.

    • Ensure that technology is maintained and available for use while conducting investigations (camera on cellphone, cellphone and laptop).

    • Willingness to provide coverage for initial assignment of cases.

    • Other duties as assigned to support the mission of the agency.

    Requirements:

    • Office of Developmental Programs (ODP) Certified Investigator Training Course – Maintain the 3-year Certified Investigator requirements and pass the recertification when due.

    • Professional experience working with individuals with developmental disabilities, their family members and other service professionals.

    • Act 33/34 and FBI clearances will be required.

    • Valid driver’s license and proof of auto insurance.

    • Bachelor’s degree in human services or related field preferred.

This person will be responsible for providing a variety of internal and external development activities to enhance the capacity of the agency and nonprofits in the community. Assess organizational needs and will design and deploy various leadership and organizational development strategies, tools, and processes that facilitate improved organizational performance. This position will work collaboratively with the agency management team or external organizational management to develop and deploy solutions.     Responsibilities:

    • Performs leadership development and capacity building activities with educational and community partners to other nonprofits through workshops and ongoing management consulting and support activities. May develop and deliver educational workshops and related activities.

    • Provides grant writing and program development services to include identifying opportunities, completing and submitting grant and funding applications, completing required reports, and supporting grant writing for internal projects and contracted external organizations.

    • Build relationships with foundations and key community partners.

    Qualifications:

    • Minimum of a bachelor's degree.

    • Three to five years of work experience in technical writing, grant writing/fundraising, and nonprofit experience.
